Appointment Request

Do you need to see one of our board-certified pediatric neurologists right away? New patient appointments are typically available in two weeks or less from the time you contact us. Please become familiar with our appointment policies and fill out the form below letting us know your preferred date and time or call us at (407) 649-1848 during our business hours. We make every attempt to be responsive to your scheduling needs. Once you fill out the appointment request form, one of our administrative staff will contact you promptly to confirm the date and time of your appointment.


Appointment Policies

We strive to provide excellent medical care to you and to all of our patients. Keeping regularly scheduled medical appointments is of utmost importance as it helps us achieve optimal healthcare outcomes while avoiding negative, sometimes severe, consequences if missed. Please be kind and inform us as soon as you become aware that you will not be able to keep your scheduled appointment. This allows us to use your time slot to help another patient from our wait list, while we reschedule your appointment for a more convenient time and date. We sincerely appreciate your assistance and cooperation.

Our practice requires at least a 24-hour notice for cancelling or rescheduling an appointment. Please notify us by responding to reminder texts and phone calls you receive ahead of the appointment, calling our office at (407) 649-1848, using the contact us section of this website, or emailing us at [email protected]. The time we speak to you, or the time stamp of your voicemail, email, or response to text reminders will be used to determine compliance with our 24-hour notification policy. 

Our Same Day Cancellation, Same Day Reschedule and No-Show Policy is as follows: 

We request that you please give our office at least a 24-hour notice in the event that you need to reschedule or cancel your appointment. If you provide us less than a 24-hour notice to cancel, reschedule, or a no show for your appointment, your dedicated appointment time slot cannot be used to help another patient in need. A patient who is a no-show, same day reschedule, or same day cancellation, three times in a year from the date of the first occurrence of such event may not be rescheduled for future appointments and may be dismissed from our practice.

Our Late Arrival Policy is as follows: 

We kindly request that all our patients plan ahead and arrive to our practice 15 minutes prior to their appointment time. This allows us to get you properly checked in and update any information needed for your appointment. As soon as you become aware that you are running late, please notify our office immediately. If a patient is more than 15 minutes late to their appointment, the patient will forfeit their dedicated time slot. If this occurs, you may choose to wait until a provider has an opening in their schedule. Patients that are consistently late for their appointments may be subject to dismissal from the practice.
